Transitional Community Living for people recovering from Acquired Brain Injury (ABI/TBI)
To establish a transitional living environment that helps people with brain injuries transition into the highest level of independence.
To create an acquired brain injury transitional living program that allows individuals to reintegrate into their community.
Having individuals transition out of this program and live back in the community is the focus of this program. One way of doing this is through learning public transportation, money management, cooking, and medication management.
A therapeutic team will work with an individual to develop meaningful goals with the focus of transitioning back into the community. If you know someone living with a brain injury who has goals, our program has a creative way to help meet them together. This type of program model is unique, and one of the only programs in Oregon who has a residential transitional program for those living with an acquired brain injury.
